Determinants of Occupational Stress among Nurses at Kermanshah University of Medical Sciences and Its Relationship with Organizational Commitment and Psychological Well-Being

Abstract

This study aimed to explain the structural relationships between occupational stress and organizational commitment, with particular emphasis on psychological well-being, and to develop an appropriate model for nurses at Kermanshah University of Medical Sciences. This applied-developmental study employed an exploratory sequential mixed-methods design consisting of qualitative and quantitative phases. In the qualitative phase, grounded theory based on the Strauss and Corbin approach was used, and semi-structured interviews were conducted with 11 experts from Kermanshah University of Medical Sciences selected through purposive sampling until theoretical saturation was achieved. Following open, axial, and selective coding, the components of the proposed model were identified and used to develop a 35-item researcher-made questionnaire. In the quantitative phase, the questionnaire was administered to 310 nurses and nurse aides working in hospitals affiliated with Kermanshah University of Medical Sciences who were selected through simple random sampling. The proposed model was subsequently evaluated using structural equation modeling (SEM) in LISREL. The goodness-of-fit indices demonstrated an acceptable fit of the structural model (χ²/df=1.76, RMSEA=0.039, GFI=0.94, AGFI=0.96, NFI=0.95, and TLI=0.96). Payment and compensation systems (β=0.43, t=6.11) and opportunities for growth and promotion (β=0.54, t=7.51) had significant positive effects on organizational commitment. Organizational commitment (β=0.65, t=8.83) and professional human-resource capability (β=0.29, t=2.12) significantly and positively affected psychological well-being, whereas occupational stress had a significant negative effect on psychological well-being (β=-0.48, t=-5.49). Psychological well-being, in turn, had significant positive effects on human-resource development (β=0.77, t=9.80) and organizational productivity (β=0.73, t=9.21). Improving compensation systems, expanding opportunities for professional growth, strengthening professional capabilities, and reducing occupational stressors may enhance nurses’ organizational commitment and psychological well-being, which can ultimately contribute to human-resource development and greater organizational productivity within healthcare organizations.
